First Visit
On your initial visit, first we listen to your concern and reason for your appointment. Secondly, we do a check up with full mouth or just necessary X-rays. Thirdly, we do a comprehensive and detail radiographic and clinical exam of the soft tissue (the gum) and the hard tissue (the teeth). Finally, we discuss with you the different treatment options available and the ideal treatment plan for you.

Oral cancer Screening
According to the (American Cancer Society), 34,360 new cases of oral cancer were detected in 2007 in United States which resulted in 7,550 deaths. The most common risk factors are excessive use of alcohol, smoking and chewing tobacco. If Oral Cancer is detected at an early stage, it can easily be removed and the survival rate is greater than 80%. For later stages, radiation and chemotherapy are needed. Most early oral cancer detections are done by dentists during a routine examination. At Forever Smile, we use FDA approved Vizilite® Plus TBlue 360 to help us detect oral cancer at its’ early stage.

Tooth Color Fillings
Composite resin is a mixture of microscopic ceramic and glass particles dispersed in a plastic resin that can be bonded to teeth. The material comes in a wide range of shades and can be molded and sculpted to create almost any shape or contour. The fact that the resin is bonded means a seal is formed to reduce any chance of leakage, which is common with silver fillings.

Crowns and Bridges
Crown is for a damaged or root canalled tooth that cannot be restore with filling. A bridge consists of a few crowns and is a solution for replacing a missing tooth. It is an alternative solution to implant. Crowns and bridges are made of Porcelain Fused to Metal (PFM), Porcelain Fused to Gold (Captek®), or just Gold. Pure Porcelain crown and bridge is for the optimal cosmetic desiring patients.

Gum Therapy
Gum therapy is necessary for those who have Periodontitis, otherwise known as gum disease. Periodontitis starts by bacterial infection of the periodontal tissue which consists of the Gum, Ligaments, and the underlying Bone. It is a silent disease which means there is no pain until it is well advanced which leads to extraction of all teeth. Periodontal disease is a serious problem, since it not only leads to loss of teeth but also has adverse effects on Cardiovascular Disease, Diabetes, and Pregnancy (Low Birth Weight and Preterm Birth).

Steps of non surgical Gum therapy By Dr. Shelyan:
(Every steps of this therapy is important for a successful periodontal treatment and maintenance of healthy gum)

  • Deep cleaning or Scaling and Root Planning (SRP) of every tooth that is involved.
  • Irrigation of underlying gum or deep pockets with antibacterial solution.
  • Placement of Localized Antibiotic (Arestin®) in deep pockets
  • Post-Op and Oral Hygiene Instructions with home-care kit.
  • Up to three times recall visits within the first six months for maintenance of healthy gum and prevention of gum disease recurrence.

Root Canal Treatment
This treatment is indicated when a small cavity that remains on a tooth and is being ignored by the patient and/or the dentist can become large enough to infect the pulp (Nerves and blood vessels) of the tooth, which run from center of the tooth to the tip of the root. It is accompanied with severe pain and can lead to abscess and tooth loss. Another indication for root canal therapy also known as endodontic therapy is accident and injury to the tooth. Endodontic is the process of debridement of the infected pulp that is inside the root. It is then filled with root canal fillings. A tooth with calcified canal or curved root needs a root canal therapy by a specialist (Endodontist). In most cases a post and a crown are necessary after endodontic is complete. Alternative to endodontic is extraction of the tooth and replacing the tooth with Implant.

Night Guard
Grinding teeth has several consequences such as shortening the front teeth, flattening the top surface of the back teeth, receding gum, sensitivity, and TMJ injury. For those who grind and clench their jaw especially during the night and sleep, custom made night guards are indicated.

Sport Guard
Losing or chipping a tooth during a simple game is very disappointing. Most accidents happen during games like Basketball, Football, Cheerleading, Hockey, Skiing, Soccer, and Skateboarding. Sport Guards are soft and easy to use and are custom made for kids and adults.

Dentures
Dentures are used to replace missing teeth. They are removable and depending on how many teeth they replace they can be either partial or complete dentures. Some dentures are metal free and made up of flexible base that fit more comfortably. The following are some designer dentures that are more aesthetic in appearance.

Extractions and Bone Graft
The last solution for a severely broken tooth that is not restorable is to remove it. An oral surgeon is needed for some difficult extractions such as fully impacted tooth or an infected tooth with an abscess. Bone grafts are synthetic bone materials that are placed inside the bone immediately after extraction. They reduce the risk of post operative complications like pain, bleeding, infection, and swelling. They also stimulate bone formation, hence speeding the healing process. In cases where implant is replacing the extracted tooth, sterilized human (Cadaver) bone is placed for a better success of the future implant.

Fluoride Treatments
In office Fluoride treatment is used to prevent cavities in a high risk patient; mostly kids. It also helps reduce tooth sensitivity.

Sealant
Teeth with deep pits and fissures are sealed to prevent cavities. Sealants are not permanent and should be applied to the same tooth couple of time a year. This procedure is mostly for kids ages 6 to 16 and is placed on permanent molars.

Implant Restoration
Implant is an ideal treatment for replacing a missing tooth or right after extracting a tooth. It acts as the root of the tooth, thus it is placed inside the bone. A crown is then placed on the implant as final restoration. If diagnosed and placed properly, implant can be functional inside the mouth forever.

Teeth Whitening
Everyone wants sparkling teeth. Whitening is a process of removing stains from the surface of the teeth. These stains can be external or internal. External stains are results of smoking, drinking coffee, soda, wine, or any other food that can stain a white shirt. Internal stains are caused of bacteria (cavities), root canalled tooth, injury to the tooth, medications (Tetracycline), and some diseases. Sometimes internally stained teeth need Veneers (Lumineers®) in addition to whitening.

Types of Whitening:
In Home Whitening: We manufacture custom-made bleaching trays that can be used to apply whitening gels to your teeth. It can be worn during the day for 1-2 hour or overnight. It takes approximately two weeks to see results.

In Office Whitening (zoom!): This procedure requires direct dentist supervision. It takes approximately one hour to brighten your teeth up to 10 shades. This procedure is the same as seen on the ABC famous show of Extreme makeover.

Combination of in home and in office whitening: Depending on the amount of stains on the teeth, and the desired shade, in home whitening may be followed after in office whitening.

Bonding
This procedure is usually to repair small chips on the teeth or to cover sensitive areas on the teeth. It is made of composite resin that is tooth color filling. It is a conservative and popular cosmetic treatment that is also very cost effective.

Porcelain Veneer and Crowns:
Porcelain Veneer is a thin shell of porcelain (0.5 mm thick) that is cemented on to the front surface of the tooth. They are routinely placed to make cosmetic changes to teeth that are discolored, worn, chipped, or misaligned. It is an absolute solution for smile makeover. Porcelain crown is a thick layer of porcelain that covers the whole tooth. They are the most cosmetically pleasing of all types of crowns especially for the front teeth.

Lumineers®:
Lumineer® is a brand of porcelain veneer that is made of Cerinate® Porcelain. Cerinate is exceptionally strong which allows the veneers to be made very thinly. In most cases they do not require teeth reduction, therefore there is no pain and/or the need for anesthesia. They are indicated in very specific cases. An initial consultation is needed to evaluate each individual case.

Silver Filling Removal:
Old silver fillings can have a crack which can cause leaking of bacteria. This can cause a recurrent cavity under the filling. Due to deep tooth cutting technique for silver placement and retention, these recurrent cavities can lead to root canal therapy and a need for crown. Broken silver fillings can be conservatively replaced with another silver filling, or composite resins (tooth color fillings), and larger silver fillings can be replaced with porcelain onlays. By using an intraoral camera for magnification, we are able to show the patients the very small and hidden cracks on TV monitor.

Laser Gum Recontouring:
Gums play an important role in the appearance and the beauty of a smile. Inflamed gums are red and swollen and unhealthy looking. The most beautiful smiles are those with healthy teeth and healthy gum. Laser gum treatment and recontouring can treat gum disease and form a healthy and correct gum line. For severe cases of uneven gum line, a periodontist is needed to perform some minor surgeries.

Invisalign®:
Invisalign is an excellent tool to straighten the teeth without the need for braces. Invisalign uses individually made clear plastic aligners to move the teeth to the correct position. Every two weeks you change the aligners until you reach the final end result; a beautiful smile. Most cases can be treated in half the time of traditional braces. An initial appointment is needed for consultation since all cases of misalignments are not candidates for Invisalign.

Inlays and onlays:
An Inlay or Onlay is a partial crown that can be made of either Gold or Porcelain. Although porcelain is more aesthetic looking, gold will last longer. They are recommended for large cavities or replacement of large old silver fillings that are not restorable with regular filling material. Impression of the prepared tooth is sent to the laboratory for the ceramist to handcraft the restoration. This procedure will take two appointments.
